First:Form No.(2) for enumeration of work injuries for establishments which employing 50 workers or more and containing average working hours per day per worker,No. of workers,Economic activity of the establishment,Gender,Age,Current occupation,Date of injury,End of treatment date,Absence from work period,How to get injured,Injury method,Injured part,Injury result,as well as Form No.(3) for occupational diseases which related to worker insurance number,Gender, Age,Occupational status,Type of occupational exposure,Period of absence from work and Treatment result as well as
Form No.(4) for serious accidents statistics including accident date,accident type and human losses (Death / Disability)
